Donna Polk, 81

Donna L. Polk, 81, of Columbia City, died at 7:05 a.m. Monday, April 29, 2013, at her home.
She was born Sept. 24, 1931, in Kokomo, a daughter of Samuel and Margurite (Lanning) Johnson.
Her formative years were spent in Greentown. She graduated from Greentown High School, attended Marion College and received her LPN certification from Indiana University Kokomo.
On Sept. 2, 1951, she was united in marriage to Pastor C. Harold Polk. They made their home in various Wesleyan parsonages including Martinsville for two years, Kokomo for 10 years total, Peru for three years, Kingsford Heights for four years, Green Center for two years and Larwill for 14 years. In 1986, they moved into Columbia City and Rev. Polk served as minister at Oak Street Wesleyan Church for four years.
She was an LPN at Kokomo Hospital, LaPorte Hospital and retired from Whitley County Memorial Hospital.
Her church membership is with Larwill Wesleyan Church and they attend Columbia City Church of the Nazarene.
Surviving relatives include her husband, Rev. Harold Polk; four children, Rebecca (John) Revis of VanBuren, Dorothy Slusher of Pierceton, David Polk of Columbia City and Martha (Mike) Russell of Columbia City; 10 grandchildren; and nine great- grandchildren. She was preceded in death by a sister, Mary Lou Hendricks.
The funeral service will be held at 11 a.m. Friday at Smith & Sons Funeral Home, Columbia City. The burial will be in Greenhill Cemetery. The visitation will be from 2 to 4 and 6 to 8 p.m. Thursday at the funeral home.
